چکیده

R esearch over the past 2 decades has suggested that significant differences exist in the action potentials of endocardial, epicardial, and mid-myocardial (M) cells that comprise the ventricular myocardium. Relative differences in the time course of repolarization of these 3 cell types, referred to as transmural dispersion of repolarization (TDR), is largely responsible for the inscription of the T wave on the electrocardiogram (ECG). Both the morphology and duration of the T wave appear to represent underlying heterogeneity in repolarization, a process initially linked to increased risk of arrhythmia and sudden death in patients with congenital long QT-syndrome (LQTS). It is increasingly apparent that repolarization heterogeneity is present in a variety of cardiovascular diseases and syndromes and is even predictive of sudden death in the general population. Furthermore, repolarization heterogeneity has also been associated with abnormalities in myocardial mechanics, a finding that may have direct implications for understanding the pathophysiology and treatment of heart failure. This review will focus on summarizing current understanding of repolarization heterogeneity, with particular focus on clinical implications.

QT-Dispersion as a potential marker in prognosis of acute myocardial infarction

Introduction: QT dispersion in a 12 lead ECG represents the heterogeneity of ventricular repolarization. The prolongation of QT-interval dispersion increases the risk of coronary heart disease, ventricular arrhythmia, and sudden cardiac death in patients with myocardial infarction. We aimed to investigate the prognostic value of QT-dispersion in patients with acute myocardial infarction. Meth...

Electrocardiographic quantitation of ventricular repolarization.

Quantification of the electrocardiographic ventricular repolarization involving the T-U wave complex is usually performed with reference to the axis of the T wave and the QT interval duration. A novel quantitative approach to improve the description of ventricular repolarization was applied to the digitized electrocardiograms of 423 normal subjects. Six electrocardiographic repolarization chara...

A complementary index for quantification of cardiac electrical dispersion: Transmural dispersion of repolarization

We read the article titled “Influence of the left ventricular types on QT intervals in hypertensive patients” by Kunisek et al. (1) published in the January issue of Anatol J Cardiol with great interest. In this welldesigned research, the authors investigated the effects of left ventricular hypertrophy (LVH) types on cardiac repolarization parameters and severity of ventricular arrhythmias usin...

The relationship between Tonic-Clonic sizures in children and increased time of ventricular repolarization

Background: Long QT syndrome (LQTS) is a disorder in which electrical cardiac ventricular repolarization is impaired. It results in an increased risk of an irregular heartbeat which can result in palpitations, fainting, drowning, or sudden death. Long QT Syndrome may present as tonic-clonic seizure or a seizure-like disorder. By taking a superficial electrocardiogram (ECG) and proper diagnosis,...

Transmural electrophysiological heterogeneities underlying arrhythmogenesis in heart failure.

Although expression of numerous ion channels is altered in heart failure (HF), mechanisms by which dysfunction at the ionic and molecular levels lead to ventricular tachyarrhythmias in HF are unknown. Previously, we found that transmural heterogeneities of repolarization play a critical role in the genesis of polymorphic ventricular tachycardia (PVT) when QT interval was prolonged in LQT2. Beca...
